ABSTRACT

Objective:To observe the clinical effect of four-point fixation in patients with posterior chamber intraocular lens dislocation.Methods:A retrospective case series study was adopted.Sixteen patients (16 eyes) with posterior chamber intraocular lens dislocation who underwent suture suspension techniques with four-point fixation in Changzhou No.2 people's Hospital from January 2015 to January 2018 were enrolled.Postoperative effects were observed during follow-up, ranging from 6 to 13 months.The preoperative and 6-month postoperative uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), corneal endothelium cell count and astigmatism were measured and the differences were compared, and the relationships between total astigmatism and corneal astigmatism or intraocular lens induced astigmatism were analyzed, and the postoperative position of intraocular lens and complications were observed.This study adhered to the Declaration of Helsinki.The study protocol was approved by an Ethics Committee of Changzhou No.2 People's Hospital (No.2015-C-012-01).Written informed consent was obtained from each patient before surgery.Results:The mean preoperative UCVA (LogMAR) and BCVA (LogMAR) were 1.09±0.24 and 0.48±0.20, respectively, which were significantly improved to 0.30±0.12 and 0.26±0.13 at 6 months after operation, respectively.And the differences were statistically significant ( t=11.782, 3.795; both at P<0.01).The preoperative and 6-month postoperative corneal endothelium cell count were (2 270±360)/mm 2 and (2 032±327)/mm 2, respectively, and the difference was not significant ( t=1.921, P=0.074).The 6-month postoperative mean total astigmatism, corneal astigmatism and intraocular lens induced astigmatism were (-1.47±0.82)D, (-1.34±0.61)D and (-0.22±0.35)D, respectively.There was a highly positive correlation between total astigmatism and corneal astigmatism ( r=0.885, P<0.05), but there was no significant correlation between total astigmatism and intraocular lens induced astigmatism ( r=-0.432, P=0.095).No dislocation, deviation or torsion of intraocular lens were observed during the follow-up.Varying degree of symptoms of iridocyclitis were observed during early stage after operation, which disappeared after treatment.There were two cases of high intraocular pressure, which were normal after treatment.No retinal detachment, choroidal detachment, expulsive suprachoroidal hemorrhage, endophthalmitis, corneal endothelial decompensation or other complications occurred during and after operation. Conclusions:There is a stable position of intraocular lens, good visual acuity and few complications after four-point fixation with suture and suspension, which is a feasible method to treat dislocated intraocular lens.

ABSTRACT

Major depressive disorder is a mental illness characterized by depressed mood, lack of engagement in pleasurable activities, anhedonia, and cognitive-behavioral disorders. Currently, traditional pharmacological treatments for depression have a delayed therapeutic onset and low treatment effectiveness. (2R, 6R)-HNK, as a key metabolite of ketamine, can not only exert rapid and lasting antidepressant effects but also has no side effects such as hallucination and addiction caused by ketamine, which has potential clinical application values. Studies have found that the antidepressant effect of (2R, 6R)-HNK is closely related to the regulation mechanism of glutamate receptor and synaptic plasticity. Besides, the changes of downstream signaling pathways include the upregulation of brain-derived neurotrophic factor (BDNF) expression, dephosphorylation of eukaryotic elongation factor 2 (eEF2), and activation of the mammalian target of rapamycin (mTOR) play a key role in the antidepressant process of the drug. Understanding the molecular mechanisms underpinning (2R, 6R)-HNK's antidepressant effects will be invaluable for the identification of targets, which will drive the development of novel, effective, next-generation pharmacotherapies for the treatment of depression.

ABSTRACT

With the increasing maturity of unmanned aerial vehicle (UAV) technology, UAVs have become an indispensable part of search and rescue activities. In this paper, the background, advantages and classification of UAVs in the field of wounded search and rescue were briefly introduced. The application scenarios and research progress of UAVs were introduced from two aspects, including wounded search and target rescue. In the wounded search process, according to the configuration difference of the UAVs configuration under the active target and the passive target, the difference of the load was analyzed. During the target rescue process, the UAV-based rescue material delivery, telemedicine assistance, and wounded evacuation were analyzed. Through the introduction of the entire search and rescue process, the development direction of UAVs in the application of wounded search and rescue was prospected.

In the field of bilingualism research, a key scientific question is how bilinguals process two language systems, particularly the effective switch from one language to another, namely bilingual code switching. With the rapid development of neuroimaging techniques, important progresses have been made in bilingual processing studies, especially in code switching. However, consensus has not been achieved regarding the mechanisms of bilingual code switching. Bilingual switching studies using neuropsychological and neuroimaging techniques have gained insights into the temporal and spatial features of the language switching process and the neurological mechanism, which provide direct evidence for the generation mechanism of bilingual code switching.

ABSTRACT

Objective To understand the tendency of negative emotional expression and its cognitive influencing factors in breast cancer survivors in China, in order to provide evidence for emotional expression intervention. Methods The qualitative research based on the health belief model (HBM) was adopted. In-depth interviews were conducted with 21 breast cancer survivors. Data were analyzed by content analysis method. Results Among 21 people interviewed, 4 were emotionally expressed and 17 were emotionally suppressed. Aware of the negative emotions, recognize the seriousness of emotional depression, the benefits of emotional expression, the less obstacles of expression and the sense of self-efficacy can promote the occurrence of emotional expression. Conclusions Chinese breast cancer survivors are not good at expressing their negative emotions.Health care workers can change the behavior of patients through the aware of the negative emotions, recognize the seriousness of emotional depression, the benefits of emotional expression,the obstacles of expression and the sense of self-efficacy.

ABSTRACT

Objective To examine the validity and reliability of the Chinese version of ambivalence over emotional expression questionnaire(AEQ) in breast cancer survivors.Methods A total of 323 breast cancer survivors were divided into 2 parts:sample 1 contains 122 breast cancer survivors for item and exploratory factor analysis,while sample 2 contains 201 breast cancer survivors for further confirmatory factor analysis,criterion validity,and internal consistency reliability.Results One-factor construct was determined and the results of confirmatory factor analysis showed that one-factor construct AEQ had a good construct validity (x2/df=1.01,CFI=0.99,TLI=0.99,NFI=0.86,IFI=0.99,RMSEA=0.02).The AEQ score was correlated with TAS-20 (r=0.28,P<0.01).Cronbach α coefficient,Spearman-Brown split coefficient and the test-retest reliability of AEQ were 0.86,0.80 and 0.79,respectively.Conclusion The AEQ is a reliable tool to assess the ambivalence over emotional expression in Chinese breast cancer survivors.

ABSTRACT

Objective To explore the relationship among emotional resilience,stress and school adjustment.Methods Adolescent life events check-list,adolescent emotional resilience scale and school adjustment scale of middle school students were conducted among 394 junior high school students.Results (1)The emotional resilience score and emotion recovery score of male junior high school students(46.07±8.56,24.43±5.80) were higher than that of female ones(44.39±8.39,23.27±5.37),and the differences were statistically significant(t=1.97,2.05,P<0.05).The total score of emotional resilience(45.46±8.50,44.83±8.56) was not statistically related with whether they were the only children(t=0.68,P=0.50).The emotional resilience score and positive emotion score of rural junior high school students(44.04±8.53,20.63±5.35)were lower than that of urban ones (46.46± 8.34,22.15 ±4.72),and the differences were statistically significant(t=-2.86,-2.99,P<0.01).The total score of emotional resilience (47.23±7.82,44.63±8.45,43.00±8.97) in different grades was statistically significant (F=8.86,P=0.00),and the first grade students were higher than the second and third year students.(2)Psychological stress was negatively related with emotional resilience and school adjustment(r=-0.23~-0.35,P<0.01),and emotional resilience was positively correlated with school adjustment(r=0.12 ~ 0.29,P<0.01).The relationship between stress and school adjustment were mediated by positive emotion and emotional recovery (the mediating effect were 11.11%,21.15%).Conclusion Frotional resilience plays a mediating role between psychological stress and school adjustment.

ABSTRACT

Human gait involves a complex mechanism of muscular skeletal coordinated operation,which is specific and can be used as the basis of identity recognitions and clinical disease diagnoses.Human gaits have wide application value in the field of disaster rescue,battlefield ambulance,counter-terrorism,security,and medical and healthcare.The traditional contact-free gait detection technology mainly depends on optical images or ultrasound,which is susceptible to light,low visibility,obstacles,etc.In recent years,with the rapidly development of bio-radar technology,the bio-radar based contact-free human gait signal detection technology has shown more advantages.It can not be affected by light,can penetrate clothing,camouflage or even walls,and can operate in all-weathe,including low visibility weather conditions such as smog,smoke and fog.In this paper,the technical principles and methods of bio-radar based contact-free human gait detection technologies were discussed,the research status was summarized,and the development trendency was prospected.

ABSTRACT

Objective To develop a fixed ruler for the simulated locator to measure the distance between human organs.Methods Analyses were carried out on the problems of the existing measures of the simulated locator and clinical requirements,and then the matching of the ruler with the locator and etc were explored.The structures of the fixing hole and slots were determined,and the ruler base was selected.The ruler was composed of an organic glass plate with multi slots and multi cylindrical copper wires.Results At the isocenter of the locator,the fixed scale values of the ruler equaled to those of the ruler on the screen,and the values for multi point distances could be obtained with the fixed ruler and stored in medical image.Conclusion The fixed ruler measures the distance between human organs simply and accurately,and lays a foundation for the development of likely product.

ABSTRACT

Physiological monitoring devices in modern clinical area are basically used electrodes or sensors directly touching the surface of human subject body, which will increase physiological and psychological load of the subjects. In order to realize non-contact monitoring of respiration and heartbeat, firstly, the micro bioradar was used to detect human body motion signal. Then, the respiration signal and heartbeat signal was extracted from the body-motion signal by using signal and conditioning circuits, digital filter and signal processing. Finally, the results of heart rate and breathing rate was wirelessly transmitted. The experimental results showed that the device for non-contact monitoring of respiration and heartbeat waveforms has advantages of small volume, low power consumption, which can realize the monitoring of physiological parameters in real time.

ABSTRACT

To separate the biological radar echo signals in different breathing modes. Echo signals of bioradar-based vital signs monitoring system were acquired in different breathing modes, and an improved signal separa-tion algorithm was used to obtain respiratory and heartbeat signals as well as their parameters. Under two breath-ing modes, the center frequencies of the signals from the self-adaptive filter were kept consistent with those of heartbeat signals, and the signals with other frequencies were suppressed effectively. The algorithm can be used to sep-arate heartbeat signals while suppress other interference signals effectively. ZHANG Jing and LIU Qian are the first au-thors who contributed equally to the article.

ABSTRACT

Objective To develop the Occupational Embeddedness Scale for Nurses and test its re liability and validity.Methods The original scale was designed according to document analysis,openlabel questionnaire and interview results.After 3 sampling tests,the first predicted sample (n=368) was conducted with item analysis and exploratory factor analysis (EFA),the second predicted sample (n=306) was conducted with EFA,and then the formal tested sample (n=372) was conducted with confirmatory factor analysis (CFA).Another sample of 55 nurses was tested and retested two weeks later.The intestine emendatory Turnover Intention Scale was used to test its criterion validity.Results The final scale had 18 items and 3 dimensions,including fit,link and Sacrifice.Occupational Embeddedness Scale revealed good model fit indices (x2/df =2.563,RMR=0.048,TLI=0.914,CFI=0.925).The total scores was negatively correlated with the Turnover Intention Scale scores.The Cronbach's α coefficients of the whole scale was 0.917,and range 0.800~0.834 for each dimension.The test-retest reliability was 0.890,and ranged 0.732~0.873 for each dimension.Conclusions It suggests that the Occupational Embeddedness Scale for Nursing has good reliability and validity,and can be used to assess the occupational embeddedness status of nursing in China.

ABSTRACT

Bioradar is based on the integration of theory of radar and bio-medical engineering,which can detect the life-parameters in farther distance.It is new concept radar presented by foreign experts.The technology can be widely used in detection of lifeform signal and non-contact clinical monitoring.Biomedical signal processing method is the premise that the technology can be realized.The signals can be interfered by the environmental factors,breath motion and so on,especially the influences of breath motion which can not be solved by average methods.A large number of signal processing methods are used in various aspects of the technology.This review introduces the progress of bioradar technology and the application of the current signal processing methods in the field.

ABSTRACT

OBJECTIVE@#To analyze the reasons for the failure of thoracolumbar spine internal fixation after burst fracture, and to discuss the strategy for revision surgery.@*METHODS@#From January 2005 to September 2010, 21 patients with thoracolumbar burst fracture received revision surgery after the failure of internal fixation. The etiology included loose of the fixation after anterior surgery in 4 patients, involving malunion with severe kyphosis in 1 patient, pedicle screw malposition in 3, fracture of adjacent segment vertebra in 2, broken of the fixation as non-union of the fracture vertebra in 10 comprising obvious kyphotic deformity in 5, and 2 had developing kyphosis for over distraction of the vertebra followed by pseudarthrosis after removing the internal fixation. After the first operation, symptoms remained in 6 patients, deteriorated in 7, light to moderate improved in 8. Surgical procedures such as anterior corpectomy and reconstruction, replacement of the internal fixation and canal decompression, vertebroplasty, and posterior pedicle subtraction osteotomy were directed based on individual situation.@*RESULTS@#Patients were followed-up for 6-68 (27.0±13.2) months, and demonstrated solid fusion 6 to 12 months postoperatively. No failure of the internal fixation reccurred. No major complications or deterioration of neurologic status were noted. At the last follow-up, the improvement of Frankel grade was 0-2 (1.3±0.7). The mean visual analog scale (VAS) dropped down from 7.6 to 2.1, and the mean oswestry disability index (ODI) decreased from 48.7 to 10.3. Preoperative angle of 7 kyphotic deformity patients was 10 degree-75 degree (42.5 degree±15.3 degree), and was improved to -3 degree-10 degree (2.3 degree±3.7 degree). At the last follow-up, the loss of correction was 0 degree-1.3 degree (0.7 degree±0.3 degree), with the correction rate of 92.3%.@*CONCLUSION@#Solid reconstruction of the anterior and middle column of the spine is the imperative procedure to prevent failure of internal fixation in thoracolumbar burst fractures. Revision surgery with reasonable strategy is beneficial and rewarding with few complications.

ABSTRACT

OBJECTIVE@#To provide further foundation for the therapy for spine cord injury by transferred mesenchymal stem cells (MSC) expressing the brain-derived neurotrophic factor (BDNF) gene.@*METHODS@#Bone mesenchymal stem cells were isolated from rats and directly cultivated and expanded in vitro. Bone mesenchymal stem cells were divided into 3 groups: a pEGFP-N1- BDNF-transfected group (BDNF group), a pEGFP-N1 transfected group (vector group) and a non-transfected group. Western blot was applied to detect the expression of transgenic bone mesenchymal stem cells. The differentiation of bone mesenchymal stem cells was identified by immunofluorescence. The rates of induction of neuron-like cells among the three cell groups were compared.@*RESULTS@#The cell surface markers of bone mesenchymal stem cells included expression of CD90 (+), CD44 (+), CD34 (-), CD45 (-). The expression of BDNF in the transfected bone mesenchymal stem cells was demonstrated by Western blot. The positive ratio of neuron-like differentiation in the BDNF group was higher than that of both the vector group and the non-transfected group (P<0.05).@*CONCLUSION@#BDNF gene plays an important role in promoting bone marrow mesenchymal stem cells to differentiate into neuron-like cells.

ABSTRACT

The measurement methods of respiration can be mainly divided into contact and non-contact.Contact detection methods include volumetric flowmeter,velocity flowmeter,temperature detection,displacement detection,impedance detection,wearable technology and sleep mattress,etc.Although these methods are noninvasive and mature,they all require electrodes or sensors to touch the body of human subjects,which limit the users’ freedom and are not suitable for severely bumed patients and newborns.In non-contact monitoring,the respiration is measured by using the media such as electromagnetic waves,optical media,etc,and without any electrodes or sensors to touch the body whiles the human subject keeping natural status.Non-contact detection method using infrared and biological radar has been a research focus both at home and abroad,which can be used for clinical,community and family care.

ABSTRACT

Objective To investigate the expressions of platelet activation-dependent granule membrane protein and platelet-derived growth factor receptor-αB, and the ultra-microstructure changes of platelets in patients with acute ST-segment elevation myocardial infarction(STEMI). Method The expressions of platelet activationdependent granule of glycoprotein (CD62P)and platelet derived growth factor receptor αβ subtype (PDGFR-αβ)of platelets in peripheral blood in 36 patients with acute ST-segment elevation myocardial infarction(STEMI) hospitalized and another 34 healthy subjects over the same period (control group) were investigated by flow cytometry and data were analyzed. The changes of ultra microstructure and activity of blood platelets in those patients and control group were observed under the scanning electron microscope. Results The expressions of CD62P and PDGFR-αβin patients with STEMI group before treatment were (3.65 ± 1.87) % and (0.43 ± 0.39) %, respectively, and those after treatment were (0.96 ± 0.79) % and (0.28 ± 0. 24) %, respectively, whereas those in control group were (0.67 ± 0.35) % and (0.27 ± 0.22) %, respectively, which were much lower in control than those in patients with STEMI before treatment (P < 0.01 or P < 0.05) respectively. There were statistically significant differences in the expressions of CD62P and PDGFR-αβ in patients group between pre-treatment and posttreatment (P <0.01 or P <0.05), respectively. Obvious ultra-microstructure changes of platelet surface in patients with STEMI group were observed. Conclusions Due to platelet activation in AMI, the expressions of CD62P can be used as effective indicators for monitoring coronary heart disease, and the PDGFR-αβ can be used as a reference indicator. The platelet surface ultra-microstructure changes during platelet activation in patients with AMI can be found by scanning electron microscopy.

ABSTRACT

OBJECTIVE@#To investigate the clinical characteristics, diagnostic foundation and treatment of vertebral sarcoidosis.@*METHODS@#The clinical data of 13 patients with vertebral sarcoidosis who received anterior debridement and instrumentation were retrospectively analyzed.@*RESULTS@#The onset of progressive pain in the pathological region was common in the 13 patients. Neurologic deficit existed in 4 cases. Radiographic study showed multiple vertebral bone destructions, and no other systemic lesions were found. Surgical indications were progressive vertebral destruction, spinal instability or neurological deficit. Anterior vertebra resection, and autologous bone grafts fusion with internal fixation were done. No operative mortality and major complications occurred. Diagnosis was confirmed in all patients by pathological exam. After the surgery, metacortandracin treatment was given routinely for 1 year. Patients were followed up for 12-52 (median 26) months, and pain and neurological symptoms were alleviated. Visual analog scale (VAS) score was 7-10 (median 8) points preoperatively, which dropped to 0-4 (median 2) points postoperatively. All patients showed successful bone fusion with no recurrence.@*CONCLUSION@#For vertebral sarcoidosis associated with progressive instability and/or neurological symptoms, surgical intervention combined with steroid therapy is safe and effective.

ABSTRACT

OBJECTIVE@#To determine the effect of transfection of enhanced green fluorescent protein (EGFP) on neuron-like differentiation of rat mesenchymal stem cells (rMSC) with plasmid vector.@*METHODS@#In vitro cultured rMSC was transfected with plasmid vector containing EGFP, and its surface marker and differentiation neuron-like cells were detected. Results There was no significant difference in the morphology and surface markers between the EGFP-rMSC and rMSC. The cell surface markers of EGFP-rMSC including expression of CD44(+),CD11b (-) and CD45(-) remained similar to those of rMSC. EGFP-rMSC presented the differentiated potential to neuron-like cells. There was no statistical difference in the positive ratio of neuron-like differentiation between the EGFP-rMSC and rMSC (P> 0.05).@*CONCLUSION@#EGFP does not affect the neuron-like differentiation potential of rMSC, and can be used as the trace marker in the study of differentiation potential of rMSC.

ObjectiveTo evaluate the learning curve of utilizing the full endoscopic technique for the surgical treatment of lumbar disc herniation.MethodsFrom August 2008 to February 2009,30 patients with lumbar disc herniation underwent lumbar discectomy by the full endoscopic-only approach were retrospectively analyzed.The patients were divided into three groups of 10 sequential cases each.Group of early time consisted of the first 10 cases,Group of medium term the subsequent 10 cases,and Group of later time the last 10 cases.The clinical evaluation data included operative time,length of hospital stay,leg and back pain visual analogue scale (VAS),and complications.ResultsAll patients were observed prospectively for (1.61±0.22) years.There was no measurable intraoperative bleeding,no postoperative infections and symptomatic recurrences in the three groups.Compared to group of early time,the operative time in group of medium term was significantly decreased(P<0.05).The patients in group of later time had much less operative time than that in group medium term(P<0.05).There was no significant difference with length of hospital stay between the three groups (P>0.05).The improvement of leg and back VAS in each group was similar:there was a significant improvement 3 months post-operation compared with pre-operation (P <0.05),but no statistical difference between 3 months post-operation and final foliow-up(P>0.05).The complication rate was 12.5% for group of early time,10% for group of medium term,and 0 for group of later time.There were two cases conversed to an open procedure for group of early time compared with 0 cases in both other groups.ConclusionExcellent clinical and minimally invasive outcomes can be obtained in the surgical treatment of lumbar disc herniation via the interlaminar approach assisted by full endoscopic technique.However,attention must be paid to the steep learning curve with use of this complex technique.Obtaining microsurgical experience and suitable patient selection can help shorten the learning curve and decrease the complications.
